Usually moisture or theres something the myc doesnt wanna roll over. Could also just be metabolites if its the very bottom of the jar thats not colonizing. I mitigate it by laying my jars down on their sides and rotate every few days or so.
Oh and for your additions. Try adding a tablespoon of light corn syrup. And azomite alongside the gypsum helps. I've personally been experimenting with Peptone lately. Terence Mckenna swore by it. Will post results when the time comes.
Bacteria contamination is often the culprit when experiencing such phenomena..... And the bacterium can gain a foothold due to overly wet conditions and/or due to lack of FAE if using very large jars with the gas exchange filter in the lid.

I don't believe that myecelium is going to leave nutrient dense grain uncolonized unless there is a competitor or possibly stalled due to inadequate gas exchange. No reason for additional nutrients, properly hydrated grain and a clean culture is all that is needed.

Bencmcp agreed... I use the pyrex dishes with the sealing snap down lids... fairly cheap and super easy to use
TheUnknown I use gallon pickle jars myself personally, they are free if you know where to look, have good lids and work better for my purposes. I still recommend pyrex with lids though.
